Unified Sponsorship, Donation & Facility Usage Request Form

Grant Reporting Requirement

If your organization is approved to receive a sponsorship, donation, or facility usage, you will be required to submit a final report within 30 days of your event or project completion. The report must include the following:

1. Impact and Potential:
   A candid reflection on the short-term outcomes and the longer-term potential of the project. Please describe how the initiative made a difference and how it may sustain positive change.
2. Challenges and Lessons Learned:
   An honest overview of any challenges faced, unexpected outcomes, or setbacks—along with key takeaways. This information is valued and has no negative impact on future consideration.
3. Summary of Activities:
   A detailed summary of completed activities, noting any changes to the original timeline, scope, or approach submitted in your application.
4. Budget Report:
   • Original Program Budget: Please include a copy of your original budget.
   • Actual Program Budget: Provide the final budget with actual expenditures.
   • Budget Variance Explanation: Describe any discrepancies between the proposed and actual budgets, including unexpected expenses or cost savings.

Failure to submit a completed report within 30 days will result in your organization being ineligible for future funding or facility use

Please note: Use of the facility includes access to the space only. All furnishings—including tables, chairs, linens, and any other event-related items—must be provided and arranged by the hosting organization. Set-up and tear-down are the sole responsibility of the event organizers.

Events with 25 or more attendees are required to have professional security and protective services. Security must be on-site from 1 hour before the event start time to 1 hour after the event ends. You are responsible for the cost of security at $55/hour, coordinated by Investment Real Estate. One guard is required for every 100 attendees. Estimated attendance must be submitted two weeks prior to the event.

By submitting this form, you agree to the terms and conditions for receiving sponsorship, donation, or facility usage from Investment Real Estate Group of Companies, MooveIn Self Storage, and The Gilliland Family Foundation, including any rules regarding sponsorship recognition, deadlines, and requirements for logos or branding.
